Your protected health information may only be used or disclosed in the course of business to provide, coordinate, or manage your healthcare and any related services, for example to fill a prescription or order laboratory tests; or when we are required to do so by federal, state or local law.

We will make every effort to respect your privacy when scheduling appointments to minimize the chance of meeting with friends or acquaintances, and you have the right to request the manner in which our practice communicates with you.
